If it does not match the plan
- Unprotected fall exposure 6 ft or greater with no guardrail/cover/personal fall arrest in use -> STOP-WORK: document, clear the area, route to the competent person; do not proceed past the exposure.
- Damaged/incomplete scaffold or a non-compliant ladder in active use -> STOP-WORK for that element; flag to the competent person, do not use.
- Temporary circuit with no GFCI, or a GFCI that fails to trip -> flag as electrocution hazard; do not touch, route to a licensed electrician / competent person.
- Any live-electrical, confined-space, trenching 5 ft or deeper, or hot-work condition -> observe-and-document only; never enter; route to the site's qualified/competent person. Approval and correction stay human.
