The playground's fall-zone surfacing has thinned below the ASTM F1292 impact-attenuation depth in several spots — a real fall-injury risk under the equipment kids use every day. Standard: undefined Proof: before/after depth measurements + photo set + GPS Method (undefined): 1. Measure surfacing depth at each piece of equipment's fall zone and photograph as found. 2. Clear debris and rake the existing surfacing material level. 3. Add engineered wood fiber or rubber mulch to restore depth to spec at each thin spot. 4. Re-measure and photograph the restored depth at each spot. Done when: before/after depth measurements + photo set + GPS

- 2. Photograph each hazard.
- 3. Classify the hazard type and assign a severity tag (minor|moderate|severe|critical).
- 4. Name the responsible handoff target (BID / city agency / utility).
- 5. Enter each hazard's type, severity, GPS, photo, and handoff target into the proof record; sign.
If it does not match the plan
- NEVER perform repair or enter the roadway/public right-of-way -> documentation and coordination only.
- Any severity:critical (e.g., live downed wire, open utility vault) -> flag immediately to the responsible agency; keep clear.
