Steps

If it does not match the plan

Atmospheric readings outside acceptable limits (O2 <19.5% or >23.5%, LEL >=10%, or toxics above the PEL), or continuous monitoring not in place -> STOP-WORK; keep out; route to the entry supervisor / a qualified industrial hygienist; never enter to re-test.
No attendant stationed outside, or no rescue/non-entry-retrieval plan and emergency-service arrangement for a permit space -> flag stop-work to the entry supervisor before any entry.
Any entry, in-space atmospheric testing, ventilation setup, or rescue -> observe-and-document only; the entry supervisor / authorized entrant / trained rescue team performs and approves these. Approval and correction stay human.
