- Outriggers/stabilizers not fully extended or on inadequate ground/cribbing, or a non-level crane -> STOP-WORK for the lift; route to the qualified operator; never operate or level it yourself.
- Swing/crush zone not barricaded, no qualified signal person, or damaged rigging (no safety latch, cut/kinked sling) -> STOP-WORK; keep workers clear; route to the lift director / qualified rigger.
- Any rigging, signaling, leveling, or lift direction -> observe-and-document only; the qualified rigger / signal person / operator performs and approves these. Approval and correction stay human.
