Steps
- 1. Check in with the entry supervisor; confirm scope, don site PPE, and confirm you observe-and-document from OUTSIDE the space only, you never enter, never perform in-space atmospheric testing, and never authorize entry (Subpart AA reserves these for the entry supervisor / authorized entrant).
- 2. Classify the space with the entry supervisor: permit-required (a potential hazardous atmosphere, engulfment, entrapment/converging walls, or another serious hazard) or non-permit; photograph the space from outside and record the classification (§1926.1203).
- 3. For a permit-required space, confirm a completed entry permit is posted: authorized entrant(s), attendant, and entry supervisor named; hazards, controls, acceptable entry conditions, and permit duration recorded (§1926.1206).
- 4. Document the atmospheric-testing log: pre-entry and continuous readings for oxygen (19.5–23.5%), flammables (<10% LEL), and toxics (e.g., H2S, CO); confirm a calibrated meter and who performed the test, you record the log, you never test inside the space (§1926.1204).
- 5. Confirm the attendant is stationed outside with continuous entrant communication, and that a rescue plan / non-entry retrieval (harness + lifeline) plus an emergency-service arrangement is in place (§§1926.1209–1211).
- 6. For each finding, mark the condition against the Subpart AA checklist item, assign a tier (stop-work / correct-before-proceed / monitor), and name the responsible entry supervisor.
- 7. Enter photos (from outside), GPS/address, the checklist, the permit + atmospheric-log status, and the tiered findings; sign. Route stop-work items to the entry supervisor with a timestamp, never enter or authorize entry.
