What the work involvesFalls are the leading cause of injury in older adults, and Tampa's large older population makes an equipment-free, graded balance screen high-value. The 4-stage progression plus tandem gait finds where balance fails before it becomes a fall.

- tape or a marked 3-meter straight line on the floor

Steps
- 1. Screen first: confirm the person can stand and walk unaided and is not acutely dizzy today; position them beside a wall/rail with a spotter within reach — the guide times and coaches only and never physically supports a fall.
- 2. Stage 1 (feet together): stand with feet side-by-side, arms at sides; time the hold up to 10 s. Progress to the next stage only if ~10 s is held.
- 3. Stage 2 (semi-tandem): place the instep of one foot beside the big toe of the other; time up to 10 s.
- 4. Stage 3 (full tandem): place one heel directly in front of the opposite toe (touching); time up to 10 s — holding <10 s here flags elevated fall risk.
- 5. Stage 4 (single-leg): stand on one leg; time up to 10 s each side (skip and record if tandem could not be held).
- 6. Dynamic component: walk the 3-meter marked line heel-to-toe (tandem gait); record steps completed, any step-off the line, and any need to grab support.
- 7. Photograph the setup; log each stage's hold time, the highest stage held, the tandem-gait result, and whether a safety grab occurred; note prior falls (last 12 months); sign it.
What counts as done
- Each of the 4 stages (feet-together / semi-tandem / tandem / single-leg) timed up to 10 s, highest stage held recorded
- 3-meter tandem-gait walk result recorded (steps completed / step-offs / need to grab support)
- Setup photographed showing a spotter present and a wall/rail within reach; any stop (grab, near-fall, dizziness) recorded with the stage it occurred
- Prior-fall history (last 12 months) and any dizziness/vertigo noted and routed to a clinician or physical therapist where flagged
