What the work involvesA strong sense of life purpose is linked to lower all-cause mortality (Alimujiang 2019), and chronic perceived stress drives inflammatory aging. Two brief validated measures — a Purpose-in-Life subscale + the PSS-10 — turn 'how are you really doing' into comparable numbers and coach the levers, while routing distress to care — self-reflection, not therapy.

Steps
- 1. Set a private, non-judgmental setting; confirm this is a self-reflection screen, not a diagnosis or therapy; ask directly whether the person is in crisis or having thoughts of self-harm right now — if yes, STOP the questionnaire and route to a crisis line / emergency immediately.
- 2. Administer the Perceived Stress Scale (PSS-10): 10 items on how unpredictable, uncontrollable, and overloaded the last month felt, each scored 0–4; reverse-score the 4 positively worded items and sum to a 0–40 total.
- 3. Administer the brief Purpose-in-Life items (Ryff subscale): agreement statements on having direction and meaning; score per the key.
- 4. Compare the PSS-10 total against population bands (higher = more perceived stress) and the purpose score against norms; note the trend if a prior score exists.
- 5. Coach behavioral levers matched to the result — for stress: sleep, activity, social contact, and a brief daily down-regulation practice; for purpose: a values reflection and one meaningful weekly commitment; frame these as skills, not a cure.
- 6. Record both scores, their bands, and any red flags; sign. Route depression signs or severe/persistent distress to a clinician, and any suicidal ideation to a crisis line / emergency, per the decision rules.
What counts as done
- PSS-10 total (0–40) recorded with the 4 positive items reverse-scored and compared to population bands
- Purpose-in-Life score recorded against norms; trend noted if a prior result exists
- A behavioral coaching plan recorded (a stress down-regulation practice + a purpose/values step), framed as skills not treatment
- Any depression sign / severe distress routed to a clinician and any suicidal ideation routed to a crisis line / emergency, with the referral recorded
