Steps
- 1. Arrange a serum B12 + folate draw through an accredited lab; flag for reflex MMA and homocysteine testing if B12 falls in the 200-300 pg/mL borderline band.
- 2. Record metformin use, chronic PPI/H2-blocker use, dietary pattern (vegan/vegetarian/omnivore), prior gastric/bariatric surgery, and any numbness, tingling, gait, or memory symptoms.
- 3. Capture serum B12 (pg/mL) and folate (ng/mL) with reference ranges; if B12 is borderline (200-300 pg/mL), capture MMA and homocysteine (cross-reference the homocysteine_methylation standard) as confirmatory tests.
- 4. Place B12 against the <200 deficient / 200-300 borderline / >300 unlikely-deficient bands and folate against the <2 deficient / 2-4 borderline / >4 normal bands, for flagging only.
- 5. Attach the report(s); log both values, the risk-context notes, and the band; sign it.
