What the work involvesUntreated periodontal disease is both a leading cause of adult tooth loss and an independent cardiovascular/stroke risk factor (Sanz 2020; Sen 2018 Stroke), yet dental-coverage gaps keep many Nashville-area adults from ever being screened. This packet pairs the validated self-audit with a coached referral into Meharry Medical College's dental clinic network -- a historically Black dental school built to serve underserved communities, whose graduates practice in underserved communities at an unusually high rate.

Steps
- 1. Set a private, well-lit space with a mirror; confirm this is a self-audit screen, not a dental exam or diagnosis.
- 2. Administer the validated self-report items (Blicher 2005): gums bleed when brushing or flossing (yes/no + frequency), any visible gum recession or teeth 'looking longer', any loose or shifting teeth, persistent bad breath or bad taste, and the date of the last professional dental cleaning or exam.
- 3. Screen for urgent red flags up front, before continuing: facial or gum swelling, fever, severe or throbbing tooth pain, or pus/drainage near a tooth — any positive routes immediately per the decision rules.
- 4. Visually inspect the gums in the mirror under good light for redness, swelling, or recession at the gumline, and note any single tooth or region that looks different from the rest.
- 5. Score the audit: any bleeding on brushing, any tooth mobility, or more than 12 months since a professional cleaning are each an independent flag; sum the flag count.
- 6. Record smoking status and diabetes status (both major periodontitis risk multipliers and part of the cardiovascular-risk context — Sanz 2020), the flag count, and one concrete named low-cost dental-access route (FQHC dental clinic, dental/dental-hygiene-school teaching clinic, or a sliding-scale program) with a next booking step; sign the record.
What counts as done
- All five validated self-report items answered (bleeding, recession, mobility, halitosis, months since last professional cleaning) with the flag count computed
- Urgent red-flag screen (facial/gum swelling with fever, severe pain, pus/drainage) completed before the record closes, with any positive routed per the decision rules
- Smoking status and diabetes status recorded as periodontitis and cardiovascular-risk multipliers
- A dentist/periodontist referral recommended with the Nashville low-cost access route (Meharry Medical College School of Dentistry, or an equivalent FQHC/sliding-scale clinic) named and a next booking step recorded
