Steps

1. 1. Identify the tree — species where possible, approximate diameter, and its location relative to structures, walkways, parking, or other targets people or property occupy.
2. 2. Walk the full base, root flare, and visible trunk for fruiting bodies: conks, brackets, shelf fungi, or mushrooms growing from bark, wounds, cavities, or the root zone.
3. 3. Photograph each fruiting body in place with its exact position (height on trunk, root-flare side, distance from base) and take a wide shot showing the whole tree and nearby targets.
4. 4. Record other visible decay indicators alongside the fungi, without diagnosing them: cavities or open wounds, cracked or loosened bark, dead or dying limbs, and insect or woodpecker activity.
5. 5. Do NOT probe, cut into, remove, or otherwise disturb the tree or the fungi — this is an external observation only, not an assessment tool.
6. 6. Note any target the tree could strike if it or a limb failed (structure, road, walkway, parking, play area) and whether the tree sees regular human/vehicle presence.
7. 7. Record findings as decay FLAGS, never a diagnosis or a risk rating, and route to an ISA Certified Arborist (or one holding the Tree Risk Assessment Qualification, TRAQ) for the actual assessment.
8. 8. Set the recheck date and compare against any prior observation for new or changed fruiting bodies.
