Steps
- 1. Before scheduling any fasting draw, ask whether the person takes insulin, a sulfonylurea, or any other glucose-lowering medication; if yes, route to the prescribing clinician for a fasting plan or a non-fasting alternative (e.g. HbA1c) before any >=8 h fast is attempted.
- 2. Arrange HbA1c and/or fasting plasma glucose through an accredited lab; record fasting duration for FPG.
- 3. Capture HbA1c (%) and/or FPG (mg/dL) with the lab reference ranges.
- 4. If a CGM is used, record 14-day time-in-range (70-180 mg/dL), mean glucose, and time-below-range.
- 5. Place HbA1c against thresholds (normal <5.7%, prediabetes 5.7-6.4%, diabetes >=6.5% per ADA) for flagging only.
- 6. Attach the lab report / CGM export; log values, units, and fasting state; sign it. No medication or dose guidance.
- 7. If in the prediabetes band, note lifestyle levers (Zone-2 volume, resistance training, added-sugar reduction) as coaching, routing diagnosis/treatment to a clinician.
