What the work involvesBaltimore is where the real evidence for this standard's whole reason to exist was generated: the ACHIEVE trial (Lin FR, Pike JR, Albert MS, et al. Lancet. 2023;402(10404):786-797), run by co-principal investigators Frank Lin and Josef Coresh at Johns Hopkins' Cochlear Center for Hearing and Public Health, randomized 977 older adults with untreated hearing loss to a hearing-intervention program or a health-education control and followed them for 3 years. The overall trial found no significant effect, but a pre-specified higher-cognitive-risk subgroup of 238 participants showed a 48% slower rate of cognitive decline with hearing intervention -- honest results this packet states as they were published, not inflated. The 2024 Lancet Commission on dementia (Livingston et al. 2024, Lancet 404:572-628) still names hearing loss the single largest modifiable midlife dementia risk factor (population attributable fraction 7% of 14 factors, 45% combined preventable fraction). A validated audiometric screen catches hearing loss early, in the city where the strongest trial evidence for acting on it was built.

- 2. Present pure tones across standard frequencies (e.g., 500-4000 Hz) to each ear separately.
- 3. Record the lowest audible threshold per frequency per ear.
- 4. Classify pass/refer against WHO grade thresholds.
- 5. Log the threshold profile, conditions, and pass/refer; route a refer to audiology; sign it.
