Steps

Measure and photograph the failed panel (lift height, cross-slope, ponding); confirm the right-of-way permit and 811 utility locates before cutting.
Set cones/barricades and a pedestrian detour; all work stays out of the traffic lane.
Remove the failed section using wet-cut dust controls; correct the subgrade or root cause (root-prune only per the city arborist standard).
Place open-graded base and the permeable surface to ADA/PROWAG slope and lippage limits, then verify with level photos and a water-test.
