How this work is done

A documented method, so this packet comes out the same whoever does it.

Bring

  • the 3-item UCLA Loneliness Scale on paper or phone (three items, each scored 1 hardly ever / 2 some of the time / 3 often)
  • a printed list of local connection resources (senior center, faith or community groups, library programs, social-prescribing or 211 line) and the 988 Suicide and Crisis Lifeline number
  • something to share: a meal, a walk route, a game, or the errand the person named
  • a visit log (date, duration, what was done, what was offered, the person's own words on what they want next time)

Steps

  1. 1. Arrange the visit at the person's chosen time and place; confirm any household rule (pets, entry, who else is home). Introduce yourself as a companion visitor, not a health worker.
  2. 2. Spend the first part simply present: conversation the person leads, a shared meal or a walk, or the ordinary errand they asked for. No agenda, no forms first.
  3. 3. When it fits naturally, ask the three UCLA items (lack companionship, feel left out, feel isolated) and record the total (3-9). Say it is a conversation starter, not a test.
  4. 4. Ask what connection the person would actually want (a weekly call, a group, transport to something they used to do) and offer one concrete option from the resource list; where the person agrees, make the introduction or the call together (a warm handoff), never a leaflet alone.
  5. 5. Note anything that is not a companion's to handle: signs of depression (ask the two PHQ-2 questions if the person is open to it), statements about self-harm, unsafe housing, signs of abuse or neglect, or a medical concern; route each per the decision rules below.
  6. 6. Agree the next visit or call before leaving; log the visit in the person's own words; the visit record belongs to the person.

What counts as done

  • Visit log of at least 60 minutes in person, in the person's own words
  • UCLA 3-item Loneliness Scale total (3-9) with the date
  • One concrete connection option offered, and any handoff the person accepted (who, when)
  • Every decision-rule red flag recorded as absent or routed as the rule says

The money and the proof

What is promised, and what is not.

Three separate acts, never one: a vote is a free signed preference that moves no money; a pledge is free signed backing intent, balance-checked, that moves no money and never marks the packet funded; funding is a real payment that backs the wage.
