What the work involvesUnpaid family caregivers under strain die sooner (Schulz & Beach 1999) and age faster under chronic stress (Kiecolt-Glaser 2003; Epel 2004). Respite is the support they most lack (NASEM 2016), and it is hospitality: someone keeps the cared-for person company so the caregiver can rest.
How this work is done
A documented method, so this packet comes out the same whoever does it.
Bring
- the ZBI-4 screening version on paper or phone (four items, each scored 0 never / 1 rarely / 2 sometimes / 3 quite frequently / 4 nearly always; total 0-16)
- a printed list of local caregiver-support resources (Area Agency on Aging or its equivalent, caregiver support groups, formal respite programs, 211) and the 988 Suicide and Crisis Lifeline number
- a meal or groceries for the household, and whatever the cared-for person enjoys (a game, music, a walk if they are steady on their feet)
- a visit log (date, hours of relief given, what the caregiver did with the time, the ZBI-4 total, anything routed)
Steps
- 1. Arrange the block at the caregiver's chosen time; confirm the cared-for person's routine, what they enjoy, and the boundaries below (no medication administration, no lifting or transfers, no personal care).
- 2. Take over companionship and supervision of the cared-for person so the caregiver leaves, rests, or does what they choose; the relief is the point, so protect the full block.
- 3. Share a meal or bring groceries; do the ordinary household help the caregiver named (dishes, laundry, a tidy).
- 4. Before the caregiver leaves or on return, ask the four ZBI-4 items in conversation and record the total (0-16); say it is a way of noticing how heavy the load is, not a test.
- 5. Offer one concrete support (a standing weekly respite slot, a caregiver group, a formal respite program, benefits counselling) and, where the caregiver agrees, make the call or introduction together.
- 6. Note anything that is not a respite visitor's to handle (see the decision rules) and route it; log the visit; the record belongs to the caregiver.
What counts as done
- At least 3 hours of relief given, logged in the caregiver's own words
- ZBI-4 total (0-16) with the date
- One concrete support offered, and any handoff the caregiver accepted (who, when)
- Boundary list kept (no medication, no transfers, no personal care) and every red flag absent or routed as the rule says

What is promised, and what is not.
Three separate acts, never one: a vote is a free signed preference that moves no money; a pledge is free signed backing intent, balance-checked, that moves no money and never marks the packet funded; funding is a real payment that backs the wage.
