Steps

4. Visually inspect the gums in the mirror under good light for redness, swelling, or recession at the gumline, and note any single tooth or region that looks different from the rest.
5. Score the audit: any bleeding on brushing, any tooth mobility, or more than 12 months since a professional cleaning are each an independent flag; sum the flag count.
6. Record smoking status and diabetes status (both major periodontitis risk multipliers and part of the cardiovascular-risk context — Sanz 2020), the flag count, and one concrete named low-cost dental-access route (FQHC dental clinic, dental/dental-hygiene-school teaching clinic, or a sliding-scale program) with a next booking step; sign the record.
