1 · The need becomes a non-medical work packet

A community-health worker (or their agent, via x402: health-packet $0.05) drafts the packet: produce-and-staples delivery + CalFresh application assist, ZIP 90042. It's scoped as non-medical by construction, the machine gate rejects any packet that embeds a clinical decision, routing that to a clinician instead.

2 · A trained community member claims it

The packet routes to a community health worker already in the network, claim is signed by their wallet (browser wallet or passkey; no new accounts, no app to install, no PHI stored on-chain, only a hashed reference). Funding draws on existing community-benefit dollars: pay follows the proof, never a promise.

3 · Proof of delivery, checked deterministically

The worker closes with a photo of the delivered box at the door + member signature + GPS + timestamp, and a confirmation the enrollment form was submitted. A deterministic proof-checker (agent-payable, $0.01) returns the verdict: no proof, no settlement.

4 · Settlement writes the outcome receipt

Approval settles on Base, a public, permanent, PHI-free receipt: this social need, this ZIP, this intervention, delivered this week. It appends to a verified community-health outcome record Kaiser's Thrive reporting and Food Is Medicine grants can cite, and where the work is ecological (a heat-safe home retrofit, a community garden), it carries the tCO2e outcome into the carbon record too.
