Steps

1. 1. Confirm the assignment with the on-duty nurse before entering: which patient, why (fall risk, delirium risk, wandering risk, or simply alone/frightened), and any patient-specific precautions (e.g. a documented swallowing/aspiration risk, fall precautions, isolation precautions) — a sitter never self-initiates a session nursing has not assigned.
2. 2. Introduce yourself as a companion, not a clinical team member; sit with the patient and let them lead the conversation where they are able to.
3. 3. Reorient gently and naturally: the date, day of week, where they are, and who is on their care team, using the room whiteboard as a prompt — never quizzing or testing the patient.
4. 4. Where the patient can engage, offer a cognitively stimulating activity (discussing current events, structured reminiscence about their own life, a word game or cards); stop if the patient is tired or uninterested.
5. 5. Watch for and never act on anything clinical (see the decision rules) — a sitter's job is presence and alerting, never assessment or intervention.
6. 6. Log the session (duration, what was done, who assigned it) at the end; hand off any observation the next shift or nursing should know.
