What the work involvesHospital disorientation has been a documented problem since the foundational wayfinding research of the 1980s (Carpman & Grant), and a 2025 study puts the disorientation rate at up to 30% of visitors, with missed appointments and delayed consultations as real consequences. Any care organization can name this exact packet for the two moments people most often get lost: arrival and discharge.

- a facility map or floor directory, and the day's own wayfinding signage as reference
- a written follow-up card for discharge escorts: the appointment date, time, place and who to call to reschedule
- the facility's own emergency-response activation method (call button, phone extension, or in-person alert to staff) confirmed at the start of a shift, and the 988 Suicide and Crisis Lifeline number, the 211 referral line, and — depending on state and facility type — the state Long-Term Care Ombudsma

Steps
- 1. WELCOME: meet the arriving person at the entrance, ask where they need to go, and walk them there directly rather than pointing or describing directions; introduce yourself as a welcome guide, not a clinical staff member.
- 2. Along the way, orient them to landmarks they can use again (elevator bank, a distinctive sign) so they can find their way back or return independently.
- 3. DISCHARGE ESCORT: confirm with the unit's nursing or discharge staff that the patient is cleared to leave and that transport has arrived (or is arranged) before beginning the escort.
- 4. Walk alongside the patient to their transport at their own pace; never physically support their weight or perform a transfer — if they are unsteady, stop and call for facility transport staff or nursing.
- 5. Before parting, sit or stand with the patient and confirm the follow-up appointment's date, time and place together, and write it on the card; if none is scheduled, name the gap and route back to the discharging unit's own scheduling desk.
- 6. Log the welcome or escort (who, where, what was confirmed); route anything not the escort's own to handle per the decision rules.

- Discharge escort: the patient was walked from their unit to their transport, logged with date and time
- For discharge: the follow-up appointment's date, time and place confirmed and written down together, or the gap named and routed back to the discharging unit's own scheduling desk
- Every decision-rule red flag recorded as absent or routed as the rule says
