What the work involvesThe evidence for 'food as medicine' is not aspirational: a matched cohort of medically tailored meal recipients had significantly fewer hospital and skilled-nursing admissions and lower medical spending than non-recipients (Berkowitz 2019). Anyone with a clinician's own written diet order can have this exact packet named — a real meal, made to the order, shared as company, wherever they live.
How this work is done

Bring
- a copy of the clinician's or dietitian's written diet order (condition, key restrictions, date issued) confirmed BEFORE cooking
- ingredients and cookware to prepare the specific meal that meets the order (e.g. sodium content, texture modification, carbohydrate count as named)
- a delivery container and a simple log (date, meal, how it matches the order, duration shared, what was shared)
- the 988 Suicide and Crisis Lifeline number, the 211 referral line, and — depending on setting — the state Long-Term Care Ombudsman Program or Adult Protective Services line, printed for reference
Steps
- 1. Confirm the diet order directly before cooking: who wrote it, what condition or restriction it names, and the date it was issued. If there is no current written order, or it is unclear or expired, do not proceed — route back to the person's own clinician or dietitian for a current order.
- 2. Prepare the meal to match the order exactly — never substitute an ingredient, adjust a restriction, or add something the order does not call for, even if the person asks for something different.
- 3. Deliver the meal to the person's own home or care-home room at the agreed time.
- 4. Share the meal as company: sit and eat with, or alongside, the person, with unhurried conversation they lead.
- 5. NEVER place food or utensils directly in the person's mouth — if they cannot self-feed, or show any coughing, choking, or swallowing difficulty, stop and route to their own clinician or a trained feeding-assistance program immediately.
- 6. Log the visit (how the meal matched the order, duration, what was shared) and route anything not the cook's own to handle per the decision rules.
What counts as done
- The diet order confirmed: who wrote it (clinician or dietitian), the condition/restriction it names, and the date it was issued — no order, no packet
- The meal prepared logged with how it matches the order (e.g. sodium content for a low-sodium order, texture for a modified-texture order)
- The meal delivered and shared as company, with duration and what was shared logged in the person's own words where possible
- Every decision-rule red flag recorded as absent or routed as the rule says

What is promised, and what is not.
Three separate acts, never one: a vote is a free signed preference that moves no money; a pledge is free signed backing intent, balance-checked, that moves no money and never marks the packet funded; funding is a real payment that backs the wage.
